Changes in Health Status or Medications

To provide the best possible care for your child, it is essential that the school health office is kept up to date on your child’s medical needs.

Please notify the school nurse immediately if:

• There is a change in your child’s overall health status or a new diagnosis.

• A new medication has been prescribed for your child.

• A current medication has been discontinued or the dosage has changed.

Why this is important:

• Emergency Preparedness: Having accurate medical information allows the nurse to act swiftly and effectively in the event of an emergency.

• Daily Care & Assessment: Knowing your child's current treatment plan helps the nurse properly assess symptoms if your child feels unwell during the school day.

• Monitoring Side Effects: It allows the nurse to keep a watchful eye for any potential side effects that may occur while your child is at school.
